It’s easy to join Civic as a business, nonprofit or fire and rescue organization. Simply collect the documents shown and follow the prompts to upload your information in the Civic Application Status Center.
Current members can open additional deposit and loan accounts within mobile and online banking.
Sole Proprietorship
- Copy of Social Security card, if using Social Security for reporting
- Copy of EIN letter, if using EIN for reporting
You will also need ONE of the following:
- Fictitious Name Certificate or Statement
- Certificate of Assumed Name
- Business license
- Registration of trade name
Corporation
- EIN letter or business tax return
- Articles of Incorporation
- Bylaws
- Board Resolution
- Beneficial Owners Form
Limited Liability Corporation
- Copy of EIN letter or business tax return
- Articles of Organization
- Operating agreement
- Beneficial Owners Form
Partnership
- EIN Letter or business tax return
- Partnership agreement or Limited Partnership Agreement (LLP only)
- Beneficial Owners Form
